Credential Form for House of Delegates Meeting

Please certify your eligibility to vote and indicate whether you expect to attend the meeting.

• If you are an OSB Section Chairperson or County Bar Association President and will need an alternate, please leave your name in the Delegate Name box and select the first certification, then designate your alternate at the bottom of the form. After the OSB declares your alternate eligible, your alternate will then be directed to fill out this credential form using the second certification.
• If you are an elected delegate who needs to correct a previous submission, please email [email protected]. Alternate certification cannot be changed.
